"Unless there is substantial action under a conditional use permit within a maximum period of one year of its approval from the land use authority, the conditional use permit shall expire. The land use authority may grant a maximum extension of six months. Health Department has concerns about the site plan. Onsite wastewater code requires "b. For non-residential facilities, one-half of the buildable area of the lot must be available for the absorption system and replacement area." The site has been evaluated and found feasible for a septic system to receive regular bathroom waste, floor drains which may carry automobile contaminates will not be permitted. The location of the septic drainfield must be within 50ft of the evaluated test pit, see the attached test pit file for reference.

Before I can do a review I need a plan with contours and elevation data for the site. Also more detail on the frontage of the site which shows the existing asphalt and a plan and profile of how the frontage will be. There are some additional comments in the original Engineering review that still need to be addressed.
