I received another call from Carry at Phoenix Loss Control regarding the insurance for ZPSCO. I emailed it to her again.

Angela has requested a different type of invoice than what they sent us, so I emailed them asking for the invoice.

Pentrex completed their portion of the work over the weekend and have requested payment.

Dale with Penetrx called today and asked to be notified as soon as the gutters were swept. He will get the concrete sealed at that point. I spoke to Kelly, he said he'd look at it next week and see if it's ready.

Mike Hewit called and told me there is still too much snow for him to apply his product. He will keep watching and let me know as soon as he can do it. He asked about contact the neighbors, I told him to please do so when he was ready. He asked me to send over my bid package again.

I received an email yesterday from ZPSCO giving me "a heads up" on the claim for comcast

I just received a call from Kerry with Phoenix Loss Control on behalf of Comcast. She was calling about a claim filed by Comcast on a line that was cut during the installation of sidewalk in Uintah by our hired contractor, ZPSCO. She said she'd just gotten off the phone with Kim Smith, he admitted cutting the line and made sure she knew there was nothing the county could about it being as he's been paid. He was rude and cussed and screamed at her and said there was no way he was going to pay for it. She asked me to send her a copy of their certificate of liability insurance, which I did.

Mike just called and said he's looking at maybe this weekend or next weekend because it's been so cold. I told him that was fine and to keep me posted.

I sent a follow up email to Mike Hewitt to see if they've had a chance to apply their product or of the weather has been an issue for them.

Sent a follow up email to Dale at Pentrex today about the paperwork I sent him on November 28th.

ZPSCO has been paid. I'm waiting for Pentrex to sign the Notice and send it back to me.

Project was completed yesterday. I will do a final walk through and get a punch list put together. Also the contractor sent me the truck tickets and new invoice for the change order, I'm waiting for Jared to approve it. I put together a notice of Award and Notice to Proceed for Pentrex to do the sealing on the concrete 30 days from now.

Contractor has ran into areas of bad material and are having difficulty getting compaction. After a phone call with Jared it was decided they would over excavate the soft spots and place structural fill. I sent Kim an email authorizing this extra work. Contractor call me this morning and let me know he hit a gas line. I gave him a phone number for Questar.

I just spoke to the contractor, they will be moving in and starting tomorrow.

I received the signed Notice to Proceed today. Contractor indicated he will start this week when the weather turns good.

I spoke with dale from Pentrex today. He received my drawings, went out and measured the concrete and will be putting together a bid and emailing it to me to get approved. He said he was charging us $0.23 per foot.

On the 10th, I sent Dale at Pentrex a copy of all the drawings. Today he called and said he didn't receive them. I verified his email address and sent them to him again.

I did receive all the required documentation prior to the deadline. I will be setting up a pre-construction meeting in the next day or two.

I emailed ZPSCO today to remind them their performance and payment bonds are due by 5:00 today.

I received the signed Notice of Award today.

Bid was awarded to ZPSCO today.

Bid package was delivered to purchasing today.

Drawings and Bid documents are done, waiting for CRS to review before sending out to bid

I just spoke to Kelly, he said he will be able to do the asphalt on this project.

I've heard back from Dan, he said I am okay to proceed as far as my budget is concerned. I've sent pdf's to Kelly to review and the bid documents to Jared to send to CRS for review.

Drawings, bid documents and cost estimate are done. Waiting for additional information from Dan Olsen and Kelly Hipwell before sending out to bid.

Today I received the field data on the 5150 S cul-de-sac from Larry

I have looked at Mr. Decker's sidewalk and it is not a trip hazard. I did send him a letter last year explaining that I couldn't get it fixed until I fixed the trip hazards in the county but that I would put it on my list of things to be looked at. I will go look at it again this year and see if anything has changed.

I just got a call from Amy Joe in the Commission Office. She said there is a man by the name of Wayne Delker that has been trying to get his sidewalk repaired for 3 years. He said that his sidewalk has sunk. I'm not sure if there is a trip hazard, or if it's just sunk. His address is 2394 E 6150 S, and his phone is 801-479-4594.

I received a .csv file from Larry today with the centerline information on Skyline Drive and 5950 South

Larry sent Trevor out this morning to get a couple crown shots for me on the Skyline 5950 S Intersection. I asked Larry to survey the cul-de-sac at 2025 E 6175 S in Uintah, he indicated it would a month to get the Cad file. Zach is measuring lengths for me today.

Under the Americans with Disabilities Act (ADA), trip hazards are defined as a change in any vertical level over 1/4".

I received survey data from Larry yesterday and have been working on the intersection of Skyline Drive and 5950 South. The interns have taken photo's of what they were able to find that needed repair, I'm in the process of reviewing those.

Larry has been working on surveying the curb & gutter surrounding the failed waterway at 5950 S Skyline Drive in Uintah. I spoke to him about it this morning, he indicated he has roughly 200' of field work left to complete and is hopeful to get to it in the week or so. I will be walking the streets in Roy and Uintah to find areas in need of repair as I haven't had any complaints this year.
