Application

The purpose of this project is to repair the damages sustained to the roadway and storm drain system as a result of a land slide. The land slide has been monitored through survey data for a couple of years and it has been determined that it is now stable. We will be installing high-back curb & gutter along the North slide of Moose Hollow Drive, adding a catch basin and repairing the associated piping.

I met Kelly on site to see what is left to done on this project. he will installed the rip rap around the box and grade back of the new curb to make it look better. His department has already done the asphalt work. he asked me to talk to Thom about the box and water line which is constantly running.

After reviewing the APWA Manual of Standards and Specifications it looks as though I need to give them 120 days from issuing the final punch list. November 7, 2011 I will visit the site for additional photo's and if nothing has changed I will issue the end of contract letter.

This project was started before Miradi. A design was completed and put out to bid September 2010. Eddy Shaw Construction was awarded the contract and worked on the improvements until the weather got to severe. At that time an invoice was submitted requesting payment for the entire contract. Due to the deep snow cover I wasn't able to verify the projects completion. I paid the contractor for the portions of the project I knew were completed and with held the remaining monies. March 22, 2011 the contractor emailed me asking about the remaining monies. I indicated that there were remaining items to be finished. June 16, 2011 the contractor emailed me again asking if the project was ready to the final payment. I did a site visit, took photo's and on July 6, 2011 I emailed the punch list. I have visited the site many times since then and no more work has been done. In August 2011, I spoke with Curtis about the situation and he indicated that I should give him 90 days from the date of the punch list to notify the contractor in writing that no more monies will be paid out on this contract and that we will be completing the project ourselves. That date is October 6, 2011.
